Reporting a CSF-Recognized Training

This form allows you to notify the CSF that a recognized training activity may not meet the quality or relevance criteria required for the granting of continuing education units (PDUs).

Use this form if you believe that the training content is inappropriate, incomplete, inaccurate, or otherwise problematic, and that its recognition should be re-evaluated.

A satisfaction survey is also sent after each training declaration in your PDU record, but this form allows you to report an issue without waiting.
